About Twelve Trees Home Care

The process of developing a Care Plan to look after a client begins when we are approached to see if we can provide the care needed. Trained staff will visit a potential client in their home to assess, discuss and determine their needs. At the same time we perform a simple assessment of the home environment to identify any areas of undue risk. From this we develop a personal Care Plan. This Care Plan is continuously reviewed and changed to make sure that it continues to meet the clients needs. A personal carer is then assigned with the clients full consent, this trained carer will be able to undertake personal care services such as helping you to get up in the morning at the times that you wish, washing, dressing, toileting and bed-making. The carer can also help you to take any medicines that have been prescribed for you by your doctor, subject to certain limitations. A blister pack prepared by the pharmacist can work well for this service.
